Guardianship Services

Overview:
Guardianship services will provide monitoring for nursing home residents in addition to that which is provided by family and friends.

Benefits:
An increased sense of comfort in knowing that your loved one is well cared for. This is especially important for family members who travel extensively or reside out of state.

Criteria for Participation:
Individuals residing in nursing homes facilities within NH. The program participant, DPOA, guardian or other legally authorized surrogate decision maker will be responsible for signing releases so that Family Support Service can access basic information about the program participant. Fee: $60.00 annually based on 2 hours of services.
